AZ Herentals takes innovative step with digitization of “patient journey” from appointment to rehabilitation via mynexuzhealth

AZ Herentals is taking a huge leap in transforming the “patient journey” by introducing digital care pathways. As from this summer, patients undergoing a total knee replacement have been receiving their information completely digitally via the central electronic patient record: mynexuzhealth. AZ Herentals is the first hospital to adopt mynexuzhealth for this specific purpose.

Digitization offers significant benefits to patients through streamlined and personalized information delivery. This not only improves the quality of care, but also ensures that patients have the right information at the right time. The care pathway for a total knee replacement served as a pilot project.

Digital support from appointment to rehabilitation

The total knee replacement care pathway begins when a patient is scheduled for surgery. From that point on, they follow a standardized care plan that defines what doctors, nurses, and paramedics should do. This care plan continues beyond their rehabilitation. In the past, a patient was given a stack of documents to take home, not all of which were immediately relevant. To improve the patient’s experience and engagement, a solution was sought to provide timely and tailored information. 

Within the central electronic patient record, mynexuzhealth, it is possible to send notifications to patients in the app and/or via email. These notifications can also include (links to) videos, brochures, or web pages. Because a care pathway is standardized and in phases, it is possible to program these notifications based on the date of the procedure, so communication can be automated.
 

The central health record, always at your fingertips

Mynexuzhealth consolidates medical information in one secure, digital location. This way, patients have all important information at their fingertips, and healthcare providers can work closely together to provide the best care, supported by the right information at the right time, tailored to the patient's healthcare needs.
 
Two things are essential to this digitized flow of information: the central landing page on the website and a care pathway flyer, a summary containing all the essential information. While digital information is the standard, the hospital recognizes that not everyone has access to these resources. Therefore, a hard copy of the information remains available upon request.
 

High patient satisfaction: 8.5/10

The digital care pathway for a total knee replacement has been in use since this summer, and the hospital surveyed patients about their experience. The results showed that patients who received all the information digitally were enthusiastic about the new system.

Since then, the care pathway for varicose veins has been digitized, and the care pathway for a total hip replacement is being prepared. Together, AZ Herentals and nexuzhealth are building the future of healthcare in which the patient is truly central.
