Statement concerning your patient details

Nexuzhealth nv is a company incorporated under Belgian law, having its registered office in Hasselt (enterprise number 0667 753 542). Nexuzhealth is a joint venture between Cegeka Health Care nv and UZ Leuven.

Nexuzhealth is a supplier of software to healthcare institutions for managing Electronic Patient Records (EPD). This EPD is one centrally managed record across all nexuzhealth healthcare institutions. This means that information can be added about you as a patient in the same central record in all nexuzhealth healthcare institutions. In this way each healthcare professional involved in your treatment can access all the information needed to provide you with the best possible care.

Access to your patient record

However, this central patient record means that not everyone can access your record in the nexuzhealth healthcare institutions. Your record is protected by strict access rules that we can translate into three main principles:

  • All care providers with which you have a therapeutic link as the patient can access your record. This therapeutic link is established on the basis of planned or performed actions such as consultations, interventions and examinations. Moreover, it is limited in time, therefore it only concerns care providers who are involved in an ongoing treatment.
  • The function of a care provider determines how much they can see. A doctor will be able to see more than an administrative worker.
  • Care providers may also request access themselves to your patient record if this is necessary. The care provider must state a reason to do so and your treating care providers are informed of this. This means there is a continuous check on access to your record.

Nexuzhealth ensures that every access to or change in your record is recorded. As a patient you may request access from your healthcare institution to the audit logs in which these actions are listed.

Deleting your account on mynexuzhealth.be or the mynexuzhealth app

Mynexuzhealth is a portal that gives you, as a patient, access to your patient record. You can register to receive an overview of your appointments, data, access and connected institutions. If you no longer wish to make use of this, you can delete the mynexuzhealth app from your device or no longer register via mynexuzhealth.be.

It is not possible to delete the data in your account, as this is only a representation of your patient record that is managed by the healthcare institutions. The nexuzhealth healthcare institutions are bound by legal retention periods and are not authorised to delete your data. If you have any questions about this, please contact the nexuzhealth healthcare institution(s) you are connected to.

Changes to this policy

Nexuzhealth reserves the right to make changes to this policy. This policy was last amended on 11/1/2023.

  • Camera and audio recordings made via your device: to scan a QR code when using certain activation methods or adding photos or videos
  • Calendar: to add appointments to the user’s personal calendar (without reading the user’s private appointments)
  • Other data (for example, when you complete a hospital questionnaire as part of a consultation via the app)

 

This privacy policy is only concerned with your data that is processed by nexuzhealth itself, as the controller, in order to ensure proper functioning of the mynexuzhealth app.

You are not obliged to provide this data to nexuzhealth, but a refusal may prevent the app from working, or working properly.

  • User-agent: the user-agent is a structured description of characteristic properties of the browser used, it includes the vendor, version number, underlying operating system and any associated extensions. This enables nexuzhealth pro to unambiguously identify the browser and consequently interact correctly with our web application.
  • Front-end error logging

In this Privacy Statement, we explain how we use personal information we collect and process about you when you use the nexuzhealth pro for home nurses web application and/or the mobile application HN Companion App and for what purposes it will be used.

Nexuzhealth as processor

Nexuzhealth is a software supplier for the management of Electronic Patient Records (EPD). Nexuzhealth itself does not manage personal data of patients or healthcare providers, but works as a processor on behalf of our clients (CT Paramedics, etc.). If you have any questions about the following data you share as a user via the nexuzhealth pro web application for home nurses and/or the HN Companion App mobile application, you should contact your organisation (CT Paramedics, etc.) for this purpose.
  • Name, email address, user ID: for the creation of a user account and for identity verification
  • Address: to use the navigation option
  • Phone number: for correspondence
  • Location details: to use the card reader
  • Camera: to use patient verification via a one-dimensional or two-dimensional barcode of the eID card and for adding images to the medical record
  • Medical data: for information of the nurse in a patient’s medical record

This enables nexuzhealth pro to unambiguously identify the browser and consequently interact correctly with our web application.

Why does nexuzhealth collect your personal data?

These data are used purely to improve the stability of the application and to provide the necessary support in case of an incident.

In the event of an incident, your organisation (CT Paramedics, etc.) will provide first-line support. If necessary, Nexuzhealth will help the end user as second line support. In this context, the developers will have access to the production environment via an audited process to resolve the incident as quickly as possible.

Your data will not be used for automatic decision-making or profiling.

Nexuzhealth collects and processes your personal data on the basis of a legitimate interest. The processing is required to ensure the proper functioning of the application, with nexuzhealth collecting only the minimum amount of data.

When you apply for a nexuzhealth job vacancy, nexuzhealth processes your personal data, such as your contact details (surname and first name, e-mail address, phone number, company name and job title) and your CV. If you attach a cover letter, this will also be processed.

When you are invited for a job interview, notes will be taken in the job interview and kept.

Why do we collect your personal data?

We collect this information primarily to process your application for the nexuzhealth job vacancy you applied for.

If you are not selected for that position, we are happy to keep your information in our recruitment files on reserve with your permission should there be a new opportunity that is a good match for your profile.
