In case there are mismatches between the address of parent and child in both records, we cannot immediately rectify the link. In order to verify the parentage of the person applying for linkage, we may request that additional documents be provided. This is for patient protection and to comply with the Patients' Rights Act.

You may provide us with one of the following documents as an attachment to your request:

Family composition certificate

You can obtain the family composition certificate from your municipality. You can also access, download and print the document if you have an electronic identity card (eID):

via ‘my record‘, the online application of the National Register.

via 'My citizen profile', the online application of the Flemish government.

via the e-desk of your municipality.

You can also request a certificate in person via the Civil Affairs Department desk. More information concerning this document can be found on the Flemish government website: www.vlaanderen.be/attest-van-gezinssamenstelling.

Divorce settlement agreement (divorce by mutual consent)

This is a document evidencing divorce by consent.

You can request a copy or an extract of a divorce deed in the municipality in which the divorce was recorded in the civil status registers.

If you have an electronic identity card (eID), you can request a copy or extract of the deed online.

Birth certificate

You can also provide us with your child's birth certificate. You can add it as an attachment to the 'parent/child link' message.

If you have an electronic identity card (eID), you can request a copy or extract of the deed online:

via your municipality's e-desk.

via the 'Certificates and permits' page of 'My Citizen profile'.

You can request a copy or an extract of a birth certificate from the local authority of the place of birth, at the office of the Civil Affairs Department.

Kids-ID

Kids-ID is an electronic identity document for Belgian children under the age of 12.

Provide a photo of the front of the Kids-ID showing the names of the parents.
